A large portion of The Phantom Pain focuses on the main character, Big Boss. At the beginning of the game, Big Boss waits for nearly two years for the cure to the PSP virus, only to have it give him a new disease, a disease for which there is no cure; in an attempt to escape from the clutches of Mother Base, Big Boss fakes his own death. When Solid Snake arrives at Big Boss' island, he finds that Snake is the body Double-O that Big Boss was expecting; two years prior to Metal Gear Solid V, Snake promised Big Boss that he would find him again if Big Boss gave up the location of the Legacy Box, the metal box containing the four remaining Big Boss capsules. Snakes then decides to attempt the job.
Quiet, like the rest of the Big Boss characters, is named after an actual person, this being German therapist Josef Mengele, nicknamed \"Death's Head\" due to his involvement in the Holocaust. At The Phantom Pain, she is one of the two characters who can kill you before you see them (the other being FOXHOUND).
In Metal Gear Solid V: The Phantom Pain's main storyline, Venom Snake visits the Psycho Mantis Lab to search for his old friend Quiet. After the climactic battle, Venom Snake discovers that she is inside a capsule (a living unit of the Metal Gear System) capable of being lodged inside of weapons and sent out into the field.
In Metal Gear Solid V The Phantom Pain, when Snake returns to Mother Base, he sees Quiet sitting in the corridor with the Sniper Rifle. When she is asked by Otacon why she is there, she says she is looking for the Phantom Pain and, to prove it she picks up the rifle and fires a shot straight down, saying that she knows where it is. While Snake is distracted by this, Otacon flees. Through the course of the conversation, Snake questions her about the sounds they heard, but she explains that they are voices of the dead.
